The purpose of our health education services is to improve the overall health of our communities. We have developed our programs with the purpose to prevent illness and disease, promote healthy lifestyles, and to have a better informed community.

If you have an ongoing health condition like diabetes, depression, heart disease, arthritis, chronic pain or you are caring for someone with an ongoing health condition, this workshop is for you!
The Live Well Chronic Disease Self-Management program is a free 6-week workshop. The workshop is given two hours, once a week, for six weeks, in various community settings. This program is sponsored by the Senior Resources Agency on Aging & the CT Department of Social Services Division & the CT Department of Public Health.
